EMPLOYMENT DOCUMENTS

Stonetrust provides the following employment documents to its policyholders because we believe that controlling the frequency and cost of workers' compensation claims begins with hiring the right people and providing adequate training. (All of the following are in Adobe PDF. If you are a Stonetrust policyholder and would like a document in Word or another format, please make your request using the "Contact Us" form and the documents will be emailed to you.)

1) EMPLOYMENT APPLICATION
You should have job applicants complete an Employment Application even if they have a resume. Ask about gaps in employment and check all references.

Use the New Hire Checklist for the orientation of new employees.

3) SAMPLE EMPLOYEE MANUAL
This Sample Employee Manual may be used as a guide to assist you in developing your own. The Manual alerts employees to the standard of behavior expected in the workplace. ***You should consult with your attorney before adopting an employee handbook.***

4) SAMPLE DRUG POLICY
You are urged to consult your attorney before adopting the Sample Drug Policy.

5) SECOND INJURY FUND QUESTIONNAIRE
This Questionnaire is helpful in Second Injury Fund recoveries. You are urged to have all employees complete the Questionnaire AFTER they are hired. These documents are confidential and must be stored in a secure area. In the event of a workplace injury, give your Stonetrust adjuster a copy of the injured employee's form.
